The following Mintlyn Crematorium forms and documents are available for downloading for completion. Mintlyn will require the original downloaded document once completed and signed.
The Instructions for Cremation form is to be completed for all cremations to advise Mintlyn Crematorium of the requirements for the cremation funeral service. This form is normally completed by the Funeral Director acting on behalf of the family, but should you wish to arrange a funeral without a Funeral Director, then this form should be completed.

The following statutory paperwork is required before any cremation can take place and must be submitted to Mintlyn Crematorium office at least 3 working days prior to the date of the cremation.
Form 1 Application to Cremate a body will be completed by the Funeral Director acting on behalf of the family or the person organising the funeral.
Form 4-5 Doctors Medical Certificate will be completed by the doctor attending the deceased, and a second independent doctor. If a Post Mortem takes place the Coroner will provide Mintlyn Crematorium with the correct forms, and there is no need for a form 4-5.

At the time the death is registered the Registrar will provide a certificate for Burial or Cremation if no Post Mortem is to take place. This certificate must also be given to Mintlyn Crematorium before the cremation can take place.
The Application for Burial / Interment is to be completed to advise Mintlyn Crematorium of the requirements for the burial funeral service at the Borough Councils’ six cemeteries. This form is normally completed by the Funeral Director acting on behalf of the family, but should you wish to arrange a funeral without a Funeral Director, then this form should be completed.
At the time the death is registered the Registrar will provide a certificate for Burial or Cremation if no Post Mortem is to take place. If a Post Mortem takes place the Coroner will issue a certificate for Burial or Cremation. This certificate must also be given to Mintlyn Crematorium before the cremation can take place.

The Application to Erect a Memorial in a Council owned Cemetery is completed before any form of permanent memorial is erected on a purchased grave. Headstones are not permitted on unpurchased graves.
